A lot is said about pillows under the head. About what happens between the knees, almost nothing — and yet there lies a detail that many side sleepers know without being able to name it.
What happens geometrically when you lie on your side
Lie on your side and let your legs relax. The upper leg falls forward and down — simply because nothing is holding it. The pelvis turns with the leg, and the lower spine turns with the pelvis.
At the top, the head lies straight on the pillow; at the bottom, the body is twisted. The twist sits somewhere in between.
The same goes for the knees: they lie on top of each other, bone on bone, all night. Some people do not feel it at all. Others wake up because of exactly that.
Why some notice it and others do not
That depends on build, mobility and mattress. A soft mattress lets the hip and shoulder sink in deeper and evens out part of the rotation. A firm one does not. If you have wide hips and a narrow waist, you experience a stronger tilting movement than someone with a straight silhouette.
So the question is not whether you need something between your knees — but whether you are one of the people who feel it.

It simply fills the gap between the knees. The upper leg no longer falls forward, the pelvis stays where it is, and the bones no longer lie directly on each other.
